I am new to script-supervisor paperwork: which of its forms is supposed to hold screen direction and eyelines, and do the downloadable blank forms actually have those fields?

One 2026-07-22 vendor page describes four script-supervisor documents and puts 'Screen direction' and an 'Eyeline diagram' on the facing page (the sheet kept opposite the script page), while the continuity log, lined script and daily progress report are described without such fields. The blank downloadable logs inspected here (a textbook companion form, a Filmsourcing sheet, a copy linked from a blog that says continuity logs track 'eyelines') have no eyeline or screen-direction field at all, so on these forms the fields live in the facing page, which no inspected blank file provided. Evidence is a vendor's description plus blank-form inspection; no studio, guild or film-school facing-page form was obtained.
